Abstract Objective To evaluate the feasibility and accuracy of remote, clinician-guided biophysical profile (BPP) assessment using a handheld ultrasound device operated by lay users. Methods This prospective, non-randomized, quasi-blinded study compared clinician-guided ultrasound-based BPP scans performed remotely by lay users using the Pulsenmore ES device with concomitant standard in-clinic ultrasound examinations performed by healthcare professionals, with both assessments conducted consecutively and immediately one after the other. Agreement between modalities was assessed for individual BPP components and overall BPP scores. Results A total of 30 pregnant women were enrolled at a mean gestational age of 33.0?±?2.6?weeks. The mean maternal age was 31.4?±?4.5?years, and mean body mass index (BMI, calculated as weight in kilograms divided by the square of height in meters) was 27.4?±?5.2. Overall agreement between remote and in-clinic BPP scores was 90%. Agreement was 100% for cardiac activity, fetal movements, fetal presentation, placental location, and subjective amniotic fluid volume assessment. Agreement for fetal tone and breathing movements was 96.7 and 93.3%, respectively. Sensitivity and specificity of remote BPP assessment for detecting abnormal overall BPP findings, using the standard in-clinical ultrasound examination as the reference standard, were 92.6 and 66.7%. The mean difference in total BPP score was 0.07?±?0.64 points. Mean fetal heart rate and maximal vertical pocket measurements were comparable between modalities. Remote examinations required more time than in-clinic scans (8.0?±?3.9 vs. 4.6?±?3.3?min) but remained within recommended limits. Conclusion Clinician-guided remote BPP assessment using a handheld ultrasound device operated by lay users demonstrates high agreement with standard in-clinic ultrasound under supervised conditions and may represent a reliable alternative for prenatal surveillance in selected populations.

As healthcare systems worldwide accelerate the shift toward hybrid and home-based care, Pulsenmore continues to expand its clinical evidence base, commercial footprint, and healthcare partnerships. Recent milestones further strengthen the role of home ultrasound in modern prenatal care.

Growing Clinical Evidence

Supporting Hospital-at-Home Models

A new publication from University Medical Center Utrecht (UMCU), Netherlands, demonstrated that pregnant women successfully performed 240 clinician-guided ultrasound examinations from home, with excellent usability, high satisfaction, and successful assessment of key fetal parameters.

The authors concluded that tele-ultrasound can support hospital-at-home models by enabling care in diverse locations, supporting clinical decision-making, reducing travel burden, and further decentralizing specialist services.
Read the publication

Can Home Ultrasound Reduce Anxiety After Pregnancy Loss?

A recently published randomized controlled trial from Wolfson Medical Center evaluated physician-led home ultrasound for women with recurrent pregnancy loss. Key findings included:

Reduced emergency department visits

Lower anxiety scores

Increased maternal-fetal attachment

Expanding Access to Prenatal Care

Israel: Hybrid Care Gains Momentum

Hospital-at-Home programs are rapidly becoming strategic priorities across Israel. Hospitals including Sheba, Beilinson, and Rambam are increasingly investing in care models that extend services beyond hospital walls.

For maternity providers, these programs offer more than operational efficiency. They create continuous engagement throughout pregnancy, strengthening patient relationships and helping hospitals remain the preferred destination for delivery and ongoing care.

Poland: A New Hybrid Care Model

Pregnant women in Poland can now access a hybrid prenatal care pathway that combines clinical excellence with convenience and continuity of care. Congratulations to Cetina and Carepoint for pioneering this important advancement.

Abstract
Purpose To evaluate the effect of incorporating twice-weekly telemedicine home ultrasound sessions on maternal anxiety
and antenatal attachment in pregnant patients with a history of late pregnancy loss.

Methods In this quasi-randomized trial, pregnant patients with a previous pregnancy loss beyond 20 weeks of gestation were
randomized per day of enrollment to standard high-risk care (control) or additional twice-weekly home ultrasound sessions
(intervention). Maternal anxiety and antenatal attachment were assessed at baseline, mid-pregnancy, and final prenatal visit
using the State–Trait Anxiety Inventory Scale (STAI-S) and the Maternal Antenatal Attachment Scale (MAAS-2). The
primary outcome was the STAI-S score at the final visit. A total of 50 participants (25 per group) were required to detect a
20% difference in the primary outcome.

Results Demographics were comparable between groups. The intervention group demonstrated significantly lower STAI-S
scores at mid-pregnancy (46.7 ± 9.3 vs. 52.0 ± 9.0; p = 0.023) and at the final visit (43.6 ± 11.8 vs. 51.5 ± 11.5; p = 0.004),
and higher MAAS-2 scores at the final visit (79.5 ± 6.2 vs. 75.0 ± 6.9; p = 0.022). Attachment scores increased significantly
during follow-up (4.8 ± 7.3 vs. − 0.36 ± 8.0; p = 0.023). Emergency department visits were fewer in the intervention group
(3.1 ± 1.5 vs. 4.9 ± 3.3; p = 0.024). Regression analyses confirmed independent associations between home ultrasound use,
reduced anxiety, and improved attachment.
Conclusion Telemedicine home ultrasound significantly reduced maternal anxiety and improved antenatal attachment in
patients with prior late pregnancy loss. This reassurance strategy may also decrease unscheduled emergency visits, supporting
its integration into standard prenatal management in this vulnerable population.

We’re pleased to share key milestones as we expand access to connected, home-based ultrasound care worldwide.

From milestone to momentum

Following Pulsenmore’s FDA Clearance and Nasdaq Listing, we are moving into a new phase, from milestone to momentum. We are seeing growing commercial adoption across Maternal-Fetal Medicine practices and hospitals and the common thread is not size. It is mindset. There is a clear understanding that hybrid care is not optional, it is a capability that must be built. Practices are beginning to train the muscle of remote care, integrating at-home ultrasound into clinical workflows, whether starting small or scaling more broadly
Because starting is what makes the difference.

Clinical evidence is expanding the paradigm

At SMFM Las Vegas, new RCT data demonstrated that home ultrasound can go beyond monitoring.
Using visual biofeedback for pre-labor training, the study showed:

Shorter second stage of labor

Improved delivery outcomes

Meaningful psychological benefits

This signals a shift toward more active, patient-engaged care.

Care delivery is evolving

At Sheba Medical Center, even during wartime, women with high-risk pregnancy complications, including preeclampsia and fetal growth restriction, are not confined to hospital beds. They are at home, managed through hybrid care models that include home ultrasound.

Sheba’s telemedicine activity has grown to 11.5% in 2025, representing a deeper shift in how care is delivered, measured, and experienced. A transformation driven by teams who chose to rethink what’s possible.

System pressure is driving change

A recent BBC article highlights a challenge the NHS can no longer ignore:
In the U.K., nearly 1 in 4 sonographer roles remain unfilled, even as demand for ultrasound continues to grow. This is not just a workforce issue, it is a bottleneck in care delivery.

Rethinking traditional care pathways toward hybrid models could be a meaningful part of the solution. Those who start early will be better positioned to build the right capabilities and lead the change.

Global engagement, consistent signal

Across SMFM in the U.S., PSANZ in Australia, NJEF, MedInTechs in Paris, and BMFMS in the U.K., our team presented, pitched, and led conversations with clinicians and healthcare leaders.

Our work was also recognized on stage, with our study poster winning Highest Scoring Poster in Maternal Medicine at BMFMS, and Pulsenmore shortlisted as a Femtech Innovator at MedInTechs, highlighting our contribution to advancing digital prenatal care.

The signal is clear: healthcare systems are moving forward, and hybrid care is rapidly becoming part of real-world clinical practice.

Objective

Telehealth has become a transformative force in prenatal care, enhancing maternal satisfaction, reducing costs, and increasing efficiency without compromising safety or outcomes. The Pulsenmore home ultrasound is a handheld device connected to smartphones and guided by a mobile app, enables women to perform high-quality ultrasound scans at home. These scans are securely transmitted to clinicians, enabling a hybrid care model that combines remote monitoring with in-clinic expertise.

Here we demonstrate insights from three years of research and real-world application

Method

Clinical studies and real-world data analysis from hospitals and HMO home ultrasound services have been summarized and presented here.

Results

Several studies have validated the Pulsenmore device’s feasibility, safety, and user acceptance. A study with 100 pregnant women demonstrated its effectiveness in detecting fetal heartbeat, movement, and amniotic fluid volume (AFV), with high satisfaction. A pivotal multicenter study in the USA confirmed these results. Real-world data in Israel showed over 98% precision in detecting prenatal parameters across 170,000+ scans. In Germany, studies confirmed the device’s acceptance and image quality for assessing AFV and fetal heartbeat, showing high patient satisfaction and potential to reduce outpatient visits.

Validation studies confirmed the device’s reliability and accuracy in measuring fetal heart rate and maximal vertical pocket, as well as its capability to conduct remote Biophysical Profile tests for high-risk pregnancies.

The device’s potential was demonstrated in monitoring high-risk pregnancies, reducing anxiety and ER visits for women with previous pregnancy loss, and managing gestational diabetes. Real-world utilization data indicate successful home admission of women with high-risk pregnancies.

Conclusion

The Pulsenmore platform offers a safe, effective, and patient-centered solution for prenatal monitoring. Clinical evidence from three years of research and successful commercialization supports its integration into prenatal care.

The agreement allows both Pulsenmore and Clalit to market Pulsenmore FC directly to patients.

Pulsenmore previously reported results in June 2025 from a controlled clinical study at Beilinson Medical Center that demonstrated a strong safety profile, with high concordance between at-home measurements and in-clinic assessments.

Fertility treatment often requires frequent in-clinic transvaginal ultrasound scans and repeated visits that disrupt daily life. Pulsenmore FC is designed to offer another way, enabling eligible patients to complete follicular monitoring scans from home using the Pulsenmore FC, a patient-operated ultrasound device, and step-by-step guidance in a mobile app. Scan data are transmitted securely to the Beilinson NEXT care team for remote interpretation and clinical oversight, supporting monitoring of follicle development and endometrial thickness for accurate timing of key moments in treatment, including the trigger injection and egg retrieval, while helping reduce the need for repeated in-clinic visits during IVF and fertility preservation cycles.

“We believe this agreement represents a global breakthrough in how fertility care can be delivered,” said Dr. Elazar Sonnenschein, CEO and founder of Pulsenmore. “We are enabling a new model of care that respects patients’ time, privacy, and daily lives, while maintaining clinical oversight. We’re grateful to Clalit for their leadership and courage to break new ground for the Beilinson Medical Center and Beilinson NEXT. They are setting a new standard for professional, empathetic, and connected care from home.”

Clalit’s CEO, Dr. Eytan Wirtheim welcomed the signing of the agreement and the launch of Phase I – the pilot implementation phase for a limited group of women.

According to Wirtheim, “This service fits seamlessly into Clalit’s new strategic vision, under which ‘Medicine has left the building’: any medical service that can be provided to patients from the comfort and calm of their homes, instead of requiring visits to hospital clinics, will be offered in this way as well – provided it can be delivered at the same level of quality as in the hospital setting. I am confident that once we reach the stage of offering this service to the general public, every woman and every couple who will undergo fertility treatments – with all the emotional and physical complexity they entail – will come to appreciate Clalit’s new service. For the women and couples who choose to receive the service with us going forward, we will be proud to provide an innovative, world-leading hybrid service that makes life easier, even during the medical process.”

Pulsenmore also supplies Clalit with Pulsenmore ES, its physician-supervised home prenatal ultrasound solution, which has recently received FDA De Novo authorization. In October 2024, Pulsenmore signed a binding agreement with Clalit for an additional 25,000 Pulsenmore ES units over five years, supported by Pulsenmore’s hardware, secure image transmission, and clinical review workflows across Clalit’s care pathways in Israel.
